It's not immediately clear to me which is worse between a guy with personality and self-esteem issues but a healthy brain and a seemingly well-adjusted individual rapidly losing cognitive functions. The people for whom it's obvious are in denial about one of these facts.

Maybe he likes some of what Trump stands for. Maybe he thinks Biden is a trainwreck, like most people with eyes and ears. Believe it or not, there are people out there who aren't strictly left or right and just vote for the guy who makes most sense to them, as opposed to the party line.
