As in “N-word with a hard R”? Was this actually a thing before this “controversy”? I’ve always known the phrase “hard R” as referring to an R-rated movie that really earned its rating with lots of violence, sex and/or profanity. That’s no longer the case?

Yes and yes. Culturally here in the states its been a way to describe the R version without using it since at least the 90's.
