The USA and Canada are top tier in women's football, but the USA has come on in leaps and bounds in men's football in the last few years. They're not a joke any more, they are ranked 13th (Canada are 50th), so upper second tier I would say. You would always get sulky Americans saying 'We could easily win the World Cup if we wanted to, you're lucky all our best athletes go into our sports!' Maybe we'll find out if that's true in a few more years.

The standard and popularity of cricket and rugby in the US and Canada are also increasing fast.
